This Privacy Policy applies to information we collect:

It does not apply to information collected by any third party, including through any application or content (including advertising) that may link to or be accessible from or through the Site.

Information We Collect About You and How We Collect It

We collect several types of information from you through your use of our Site and Services, including information:
  • By which you may be personally identified, such as name, postal address, email address, telephone number, social security number, or any other identifier by which you may be contacted online or offline ("personal information");
  • That is about you but individually does not identify you; and/or
  • About your internet connection, the equipment you use to access our Site or Services, and usage details.

Information You Provide to Us

The information we collect about you through the use of our Site and Services may include:
  • Information that you provide by filling in forms on our Site. This includes information provided at the time of registering to use our Site or requesting further Services. We may also ask you for information when you report a problem with our Site or Services.
  • Create an Account. To create an account, you will provide us with your name, date of birth, gender, physical address, email address, phone number, and you may provide us with your ethnicity.
  • Request or Schedule an Appointment. To request or schedule an appointment, you will provide your name, phone number, email address, data of birth, department, whether you are a current patient, the reasoning for your appointment request, and any other information you choose to provide.
  • Patient Portal. When you sign up for a patient portal, you can provide health records, medications, allergies, and health issues, view test and lab results, provide payment information to pay your bills, and the information you provided to create an account. We use electronic medical record providers to maintain our portal. Please review each provider's privacy policy for more information.
  • Pay a Bill. To pay a bill online, you will provide us with your account number, name, and date of birth.
  • Request Submission Form. To submit a Patient Request to Access Medical Records or Authorization for Use and Disclosure form, you will provide your name, date of birth, address, phone number, and signature.
  • Contact Us. To contact us, you will provide your name, email address, phone number, address, and any other information you choose to provide.
  • Your search queries on the Site.
  • Other information you voluntarily provide to opt in to receive future communications from us.
  • In some cases, we may receive personal information about you from other sources. This includes government entities, advertising networks, data brokers, operating systems and platforms, mailing list providers, social networks and advertising and marketing partners.


Information We Collect Through Automatic Data Collection Technologies

Cookies and Other Tracking Technologies

Like many other companies, we use cookies and other tracking technologies (collectively, "Cookies"). Cookies are small files of information that are stored by your web browser software on your computer hard drive, mobile or other devices (e.g., smartphones or tablets).

Google Analytics
  • What it is: Google Analytics is a web analytics service offered by Google that tracks and reports website traffic. It uses "cookies," which are text files placed on your computer, to help us analyze how visitors use our Site.
  • What data is collected: Google Analytics collects information such as your IP address, device information, and usage data. We have enabled IP anonymization to protect your privacy.
  • How it is used: We use this data to understand and improve our Site, analyze user behavior, and optimize our marketing efforts.
  • Your choices: You can opt-out of Google Analytics by installing the Google Analytics opt-out browser add-on.

Third-Party Use of Cookies and Other Tracking Technologies

Some content or applications, including advertisements, on the Site are served by third-parties, including advertisers, ad networks and servers, content providers, and application providers. These third parties may use cookies alone or in conjunction with web beacons or other tracking technologies to collect information about you when you use our website. They may use this information to provide you with interest-based (behavioral) advertising or other targeted content.

We do not control these third parties' tracking technologies or how they may be used. Universal Opt-Out Mechanisms

We do not engage in "sales" or "targeted advertising" as those terms are defined under applicable laws and therefore our Site does not recognize the Global Privacy Control ("GPC") signal. For more information and to download and use a browser supporting the GPC browser signal, click here: https://globalprivacycontrol.org/orgs. If you choose to use the GPC signal, you will need to turn it on for each supported browser or browser extension you use.

Some internet browsers incorporate a "Do Not Track" feature that signals to websites you visit that you do not want to have your online activity tracked. Given that there is not a uniform way that browsers communicate the "Do Not Track" signal, the Site does not currently interpret, respond to or alter its practices when it receives "Do Not Track" signals.

Choices About How We Use and Disclose Your Information

We strive to provide you with choices regarding the personal information you provide to us. We have created mechanisms to provide you with the following control over your information:
  • Promotional Offers from the Company. If you do not wish to have your contact information used by the Company to promote our own or third parties' products or services, you can opt-out by logging into the Site and adjusting your user preferences in your account profile by checking or unchecking the relevant boxes or by sending us an email stating your request to info@jeromeriddledds.com . If we have sent you a promotional email, you may send us a return email asking to be omitted from future email distributions.
  • Targeted Advertising. If you do not want us to use information that we collect or that you provide to us to deliver advertisements according to our advertisers' target-audience preferences, you can opt out by sending us an email stating your request to info@jeromeriddledds.com .


We do not control third parties' collection or use of your information to serve interest-based advertising. However, these third parties may provide you with ways to choose not to have your information collected or used in this way. You can opt out of receiving targeted ads from members of the Network Advertising Initiative ("NAI") on the NAI's website.

Accessing and Correcting Your Information

You can review and change your personal information by logging into the Site and visiting your account profile page.

You may also send us an email at info@jeromeriddledds.com to request access to, correct or delete any personal information that you have provided to us. We cannot delete your personal information except by also deleting your user account. We may not accommodate a request to change information if we believe the change would violate any law or legal requirement or cause the information to be incorrect.

Your State Privacy Rights and Additional Disclosures

Depending on the state in which you reside, you may have certain privacy rights regarding your personal data. Your privacy rights may include (if applicable):
  • The right to confirm whether or not we are processing your personal data and to access such personal data;
  • The right to obtain a copy of your personal data that we collected from and/or about you in a portable and, to the extent technically feasible, readily usable format that allows you to transmit the information to another controller without hindrance, where the processing is carried out by automated means;
  • The right to delete personal data that we collected from and/or about you, subject to certain exceptions;
  • The right to correct inaccurate personal data that we maintain about you, subject to certain exceptions;
  • The right, if applicable, to opt out of the processing of your personal data for purposes of (1) targeted advertising; (2) the "sale" of your personal data (as that term is defined by applicable law); and (3) profiling in furtherance of decisions that produce legal or similarly significant effects concerning you;
  • If we are required by applicable law to obtain your consent to process sensitive personal data, the right to withdraw your consent; and
  • The right not to receive discriminatory treatment by us for the exercise of your privacy rights.


We do not sell personal data to third parties or engage in targeted advertising as defined under applicable laws. We do not use personal data subject to the Colorado Privacy Act for profiling in furtherance of decisions that produce legal or similarly significant effects concerning individuals. We may use protected health information subject to HIPAA to provide you with personalized medical care, including providing preventative care and treatment options.

To exercise your rights, please submit a request by emailing us at info@jeromeriddledds.com or by calling us at 480-991-4410. If legally required, we will comply with your request upon verification of your identity and, to the extent applicable, the identity of the individual on whose behalf you are making such request. To do so, we will ask you to verify data points based on information we have in our records. If you are submitting a request on behalf of another individual, please use the same contact methods described above. Please note the privacy rights described above do not apply to personal data that is collected, processed, or disclosed pursuant to HIPAA. Because we are subject to those laws and regulations, much of the personal data that we collect is exempt from other applicable privacy laws. Your choices and rights with respect to our use and sharing of that information is subject to our Patient Privacy Notice.

Data Security

We maintain commercially reasonable security measures to protect the personal data we collect and store from loss, misuse, destruction, or unauthorized access. However, no security measure or modality of data transmission over the internet is 100% secure. Although we strive to use commercially acceptable means to protect your personal data, we cannot guarantee absolute security. Any transmission of personal information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the Site or Services.

The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our Site or Services, you are responsible for keeping this password confidential. We ask you not to share your password with anyone.

Our Company would like to send you information about services of ours that we think you might like.
